This article briefly reviews concerns related to the “cultural colonialism” of applying Western biomedical models of research ethics to non-Western groups. The feasibility of alternate ethical models is discussed and found wanting. In practical terms, many academic researchers in the United States are funded by federal agencies and are required to adhere to Title 45, Part 46 of the Code of Federal Regulations , legislation that is clearly grounded in the Western biomedical research tradition. Consequently, the question is not whether (...) this system of ethics should be applied but rather how it can be applied most sensitively, appropriately, and wisely. Total knee arthroplasty (TKA) is a commonly implemented elective surgical treatment for end-stage osteoarthritis of the knee, demonstrating high success rates when assessed by objective medical outcomes. However, a considerable proportion of TKA patients report significant dissatisfaction postoperatively, related to enduring pain, functional limitations, and diminished quality of life. In this conceptual analysis, we highlight the importance of assessing patient-centred outcomes routinely in clinical practice, as these measures provide important information regarding whether surgery and postoperative rehabilitation interventions have effectively remediated (...) patients’ real-world “quality of life” experiences. We propose a novel precision medicine approach to improving patient-centred TKA outcomes through the development of a multivariate machine-learning model. The primary aim of this model is to predict individual postoperative recovery trajectories. Uniquely, this model will be developed using an interdisciplinary methodology involving non-linear analysis of the unique contributions of a range of preoperative risk and resilience factors to patient-centred TKA outcomes. Of particular importance to the model’s predictive power is the inclusion of a comprehensive assessment of modifiable psychological risk and resilience factors that have demonstrated relationships with TKA and other conditions in some studies. Despite the potential for patient psychological factors to limit recovery, they are typically not routinely assessed preoperatively in this patient group, and thus can be overlooked in rehabilitative referral and intervention decision-making. This represents a research-to-practice gap that may contribute to adverse patient-centred outcomes. Incorporating psychological risk and resilience factors into a multivariate prediction model could improve the detection of patients at risk of sub-optimal outcomes following TKA. This could provide surgeons and rehabilitation providers with a simplified tool to inform postoperative referral and intervention decision-making related to a range of interdisciplinary domains outside their usual purview. The proposed approach could facilitate the development and provision of more targeted rehabilitative interventions on the basis of identified individual needs. The roles of several modifiable psychological risk and resilience factors in recovery are summarised, and intervention options are briefly presented. While focusing on rehabilitation following TKA, we advocate for the broader utilisation of multivariate prediction models to inform individually-tailored interventions targeting a range of health conditions. The rise of precision medicine has led to an unprecedented focus on human biological material in biomedical research. In addition, rapid advances in stem cell technology, regenerative medicine and synthetic biology are leading to more complex human tissue structures and new applications with tremendous potential for medicine. While promising, these developments also raise several ethical and practical challenges which have been the subject of extensive academic debate. These debates have led to increasing calls for longitudinal governance arrangements between tissue providers (...) and biobanks that go beyond the initial moment of obtaining consent, such as closer involvement of tissue providers in what happens to their tissue, and more active participatory approaches to the governance of biobanks. However, in spite of these calls, such measures are being adopted slowly in practice, and there remains a strong tendency to focus on the consent procedure as the tool for addressing the ethical challenges of contemporary biobanking. In this paper, we argue that one of the barriers to this transition is the dominant language pervading the field of human tissue research, in which the provision of tissue is phrased as a ‘donation’ or ‘gift’, and tissue providers are referred to as ‘donors’. Because of the performative qualities of language, the effect of using ‘donation’ and ‘donor’ shapes a professional culture in which biobank participants are perceived as passive providers of tissue free from further considerations or entitlements. This hampers the kind of participatory approaches to governance that are deemed necessary to adequately address the ethical challenges currently faced in human tissue research. Rather than reinforcing this idea through language, we need to pave the way for the kind of participatory approaches to governance that are being extensively argued for by starting with the appropriate terminology. When ‘intelligent capitalism’ becomes the norm rather the exception what happens to labour as a factor of production and what happens to economy and society based on capital and labour? (...) The paper briefly reviews the sociology of labour from a Marxist view to examine conceptions of Fordist and post-Fordist capitalism, and explore the advert of ‘intelligent capitalism’ to pose the question concerning education. The clinical ethics propounded by Richard Zaner is unique. Partly because of his phenomenological orientation and partly because of his own daily practice as a clinical ethicist in a large university hospital, Zaner focuses on the particular concrete situations in which patients and their families confront illness and injury and struggle toward workable ways for dealing with them. He locates ethical reality in the clinical encounter. This encounter encompasses not only patient and physician but also the patients family and friends (...) and indeed the entire lifeworld in which the patient is still striving to live. In order to illuminate the central moral constituents of such human predicaments, Zaner discusses the often-overlooked features of disruption and crisis, the changed self, the patients dependence and the physicians power, the violation of personal boundaries and their necessary reconfiguring, and the art of listening. Peer review is a widely accepted instrument for raising the quality of science. Peer review limits the enormous unstructured influx of information and the sheer amount of dubious data, which in its absence would plunge science into chaos. In particular, peer review offers the benefit of eliminating papers that suffer from poor craftsmanship or methodological shortcomings, especially in the experimental sciences. However, we believe that peer review is not always appropriate for the evaluation of controversial hypothetical science. We argue that (...) the process of peer review can be prone to bias towards ideas that affirm the prior convictions of reviewers and against innovation and radical new ideas. Innovative hypotheses are thus highly vulnerable to being “filtered out” or made to accord with conventional wisdom by the peer review process. Consequently, having introduced peer review, the Elsevier journal Medical Hypotheses may be unable to continue its tradition as a radical journal allowing discussion of improbable or unconventional ideas. Hence we conclude by asking the publisher to consider re-introducing the system of editorial review to Medical Hypotheses. (shrink)

Objective To compare the coping patterns of physicians and clinical psychologists when confronted with clinical ethical dilemmas and to explore consistency across different dilemmas. Population 88 clinical psychologists and 149 family physicians in Israel. Method Six dilemmas representing different ethical domains were selected from the literature. Vignettes were composed for each dilemma, and seven possible behavioural responses for each were proposed, scaled from most to least ethical. The vignettes were presented to both family physicians and clinical psychologists. Results Psychologists’ aggregated (...) mean ethical intention score, as compared with the physicians, was found to be significantly higher =22.44, p<0.001, η2=0.37). Psychologists showed higher ethical intent for two dilemmas: issues of payment and dual relationships. In the other four vignettes, psychologists and physicians responded in much the same way. The highest ethical intent scores for both psychologists and physicians were for confidentiality and a colleague’s inappropriate practice due to personal problems. Conclusions Responses to the dilemmas by physicians and psychologists can be categorised into two groups: similar behaviours on the part of both professions when confronting dilemmas concerning confidentiality, inappropriate practice due to personal problems, improper professional conduct and academic issues and different behaviours when confronting either payment issues or dual relationships. In face of the multiple controversies surrounding the DSM process in general and the development of DSM-5 in particular, we have organized a discussion around what we consider six essential questions in further work on the DSM. The six questions involve: 1) the nature of a mental disorder; 2) the definition of mental disorder; 3) the issue of whether, in the current state of psychiatric science, DSM-5 should assume a cautious, conservative posture or an assertive, transformative posture; 4) the role (...) of pragmatic considerations in the construction of DSM-5; 5) the issue of utility of the DSM – whether DSM-III and IV have been designed more for clinicians or researchers, and how this conflict should be dealt with in the new manual; and 6) the possibility and advisability, given all the problems with DSM-III and IV, of designing a different diagnostic system. Part 1 of this article took up the first two questions. Part 2 took up the second two questions. Part 3 now deals with Questions 5 & 6. Question 5 confronts the issue of utility, whether the manual design of DSM-III and IV favors clinicians or researchers, and what that means for DSM-5. Our final question, Question 6, takes up a concluding issue, whether the acknowledged problems with the earlier DSMs warrants a significant overhaul of DSM-5 and future manuals. We are exorbitant, and rightly so, when we cut any link we may have to cosmological powers. Levinas invites us to be exorbitant by distancing ourselves from visions of metaphysics, epistemology, and theology. We begin to listen well to Levinas when we hear him inviting us to break completely with the pagan world in which the gods are simply the highest beings in the cosmos and learn to practice an adult religion in which God is outside cosmology and ontology. God (...) comes to mind neither in our attempts to think him as the creator of the cosmos nor in moments of ecstasy but in acts of genuine holiness, such as sharing a piece of bread with someone in a time of desperate need. Levinas, in short, enjoins us to be exorbitant in our dealings with one another. This book asks how the "between" of Levinas's thinking facilitates a dialogue between Jews and Christians. In one sense, Levinas stands exactly between Jews and Christians: ethics, as he conceives it, is a space in which religious traditions can meet. At the same time, his position seems profoundly ambivalent. No one can read a page of his writings without hearing a Jewish voice as well a a philosophical one. Yet his talk of substitution seems to resonate with Christological themes. On occasion, Levinas himself sharply distinguishes Judaism from Christianity--but to what extent can his thinking become the basis for a dialogue between Christians and Jews? This book, with a stellar cast of contributors, explores these questions, thereby providing a snapshot of the current state of Jewish-Christian dialogue. (shrink)

This paper begins the development of a pragmatics of emotion based on the pragma-dialectical programme, Externalization, Socialization, Functionalization, and Dialectification, applied to the emotional mode of argumentation. The first step points out a systematic equivocation within pragma-dialectics between the notion of argument and that of 'dialectics.' With this cleared, it is shown that each of the first three main assumptions can be altered to accommodate a non-logical mode of communication. However, dialectification, insofar as it is actually defining of the dialectical (...) mode, must be created anew. A defining assumption for emotionality is presented as a replacement for dialectification. (shrink)

Exploring the construct of social-responsibility orientation across three Asian and two Western societies, we show evidence that top-level executives in these societies hold fundamentally different beliefs about their responsibilities toward different stakeholders, with concomitant implications for their understanding and enactment of responsible leadership. We further find that these variations are more closely aligned with institutional factors than with cultural variables, suggesting a need to clarify the connection between culture and institutions on the one hand and culture and social-responsibility orientations on (...) the other. Intermediate constructs are required as bridges between complex behaviors and realistic models of neural circuitry. For cognitive scientists in general, schemas are the appropriate functional units; brain theorists can work with neural layers as units intermediate between structures subserving schemas and small neural circuits.After an account of different levels of analysis, we describe visuomotor coordination in terms of perceptual schemas and motor schemas. The interest of schemas to cognitive science in general is illustrated with the example of perceptual schemas in (...) high-level vision and motor schemas in the control of dextrous hands.Rana computatrix, the computational frog, is introduced to show how one constructs an evolving set of model families to mediate flexible cooperation between theory and experiment. Rana computatrix may be able to do for the study of the organizational principles of neural circuitry what Aplysia has done for the study of subcellular mechanisms of learning. Approach, avoidance, and detour behavior in frogs and toads are analyzed in terms of interacting schemas. Facilitation and prey recognition are implemented as tectal-pretectal interactions, with the tectum modeled by an array of tectal columns. We show how layered neural computation enters into models of stereopsis and how depth schemas may involve the interaction of accommodation and binocular cues in anurans. (shrink)

While many books have appeared on limited aspects of one subfield or another of brain theory and neural networks, the (...) Handbookcovers the entire sweep of topics—from detailed models of single neurons, analyses of a wide variety of biological neural networks, and connectionist studies of psychology and language, to mathematical analyses of a variety of abstract neural networks, and technological applications of adaptive, artificial neural networks. The excitement, and the frustration, of these topics is that they span such a broad range of disciplines including mathematics, statistical physics and chemistry, neurology and neurobiology, and computer science and electrical engineering as well as cognitive psychology, artificial intelligence, and philosophy. Thus, much effort has gone into making the Handbookaccessible to readers with varied backgrounds while still providing a clear view of much of the recent, specialized research in specific topics. The heart of the book, part III, comprises of 267 original articles by leaders in the various fields, arranged alphabetically by title. Parts I and II, written by the editor, are designed to help readers orient themselves to this vast range of material. Part I, Background, introduces several basic neural models, explains how the present study of brain theory and neural networks integrates brain theory, artificial intelligence, and cognitive psychology, and provides a tutorial on the concepts essential for understanding neural networks as dynamic, adaptive systems. Neurophysiological investigations of the past two decades have consistently demonstrated a deficit in sensory gating associated with schizophrenia. Phillips & Silverstein interpret this impairment as being consistent with cognitive coordination dysfunction. However, the physiological mechanisms that underlie sensory gating have not been shown to involve gamma-band oscillations or NMDA-receptors, both of which are critical neural elements in the cognitive coordination model.

Positions in dialogic dispute are presented enthymematically. It is important to explore the position the disputant holds. A model is offered which relies on the presentation of a counter-example to an inferred missing premiss. The example may be: [A+J embraced as falling under the rule; [A-] rejected as basically changing the position; or, [R] rejected as changing the proffered missing premiss. In each case the offered model indicates the next appropriate action. This article reviews claims for creativity in the economy and in education distinguishing two accounts: 'personal anarcho-aesthetics' and 'the design principle'. The first emerges in the psychological literature from sources in the Romantic Movement emphasizing the creative genius and the way in which creativity emerges from deep subconscious processes, involves the imagination, is anchored in the passions, cannot be directed and is beyond the rational control of the individual. This account has a close fit to business as a form of (...) 'brainstorming', 'mind-mapping' or 'strategic planning', and is closely associated with the figure of the risk-taking entrepreneur. By contrast, 'the design principle' is both relational and social and surfaces in related ideas of 'social capital', 'situated learning', and 'P2P' (peer-to-peer) accounts of commons-based peer production. It is seen to be a product of social and networked environments — rich semiotic and intelligent environments in which everything speaks. The article traces the genealogies of these two contrasting accounts of creativity and their significance for educational practice before showing how both notions are strongly connected in accounts of new forms of capitalism that require a rethinking of the notion of creativity and its place in schools and institutions of higher education. The article begins by providing a context in terms of a history of the knowledge economy and the historical tendency toward aesthetic or designer capitalism. (shrink)

This volume draws together this research, offering the first complete reference on implicit learning by those who have been instrumental in shaping the field. Laudan's "progress and its problems" is of two, incompatible, minds. proto-laudan argues that science is indexed to historical contexts, such that scientific rationality depends on progress and not vice-versa. deutero-laudan claims that sociology assumes "a rationality" and so misunderstands science. the latter is confused and offers no argument against sociology which does not also apply against historical approaches to philosophy of science, proto-laudan included. such tribal warfare is unprogressive, and best abandoned.
